A standard keyboard is one fixed block, which forces both hands and wrists to angle inward toward the center just to reach it comfortably. A split keyboard separates the two halves so each hand can sit at its own natural, shoulder-width angle instead.

Many split boards also add tenting — angling each half upward toward the center — which reduces forearm pronation, the twist your forearm holds to keep your palm flat on a standard keyboard for hours at a time.

What that genuinely fixes is strain specifically caused by wrist and forearm angle during typing — a documented, mechanical source of repetitive strain for people who type for long stretches every day.

What it doesn't fix is everything else. A split keyboard does nothing for a monitor set too low, a chair with no lumbar support, or a mouse arm reaching too far to the side — ergonomics is a whole-desk problem, and a keyboard is one piece of it, not a fix for everything else in the setup.

Split boards also range widely in just how split they actually are — some ship as a single connected unit with a fixed gap in the middle, while fully split boards separate into two completely independent halves connected by a cable, letting you place each half at whatever distance and angle actually suits your shoulders, not just a fixed factory gap.

Worth knowing going in: there's a real adjustment period. Typing speed commonly dips for the first few weeks on a split layout before recovering, which is worth expecting rather than mistaking for the board simply being a bad fit — most people who stick with it report the initial slowdown fully recovers, and often improves on their prior speed once the new hand positioning becomes automatic.
